1- SMS Consent Communication:

The information obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.

2- Types of SMS Communications:

If you have consented to receive text messages from Gen By Gen Health, you may receive messages related to the following (provide specific examples):

  • Data Collection of Vital Signs/Follow Up Messages:

Example: Hello Ms. Jane, this is Gen by Gen Health. Can you send me your BP readings from the weekend please?  You can reply STOP to opt out of SMS messaging from Gen by Gen Health.

  • Mental Health Status:

Example: Hello Sam, this is Gen by Gen Health. We’re reaching out to check on your mental health. Feel free to reply with any updates or if you need assistance.  You can reply STOP to opt out of SMS messaging from Gen by Gen Health.

  • Appointment Scheduling, Reminders & Transportation Assistance Inquiry:

Example: Hello John! I contacted your cardiologist and scheduled an appointment for you to see him next Wednesday at 3:30 PM. Would you like me to schedule transportation to and back from this appointment?

  • Medication Reminder:

Example: Hi James! This is a friendly reminder to take your prescribed medication as follows:

Medication Name: [e.g., Amoxicillin]
Dosage: [e.g., 1 tablet]
Time to Take: [e.g., Take 1 tablet in the morning, with or without food]

Please remember to take the medication at the recommended time. If you experience any issues or have any questions, feel free to reach out.  You can reply STOP to opt out of SMS messaging from Gen by Gen Health.

  • Consent to Contact Your Specialist:

Example: “Hi Sara, this is Gen by Gen Health. We would like to contact your Cardiologist, on your behalf to assist with your care. By replying ‘Yes’, you consent to us reaching out to your specialist. If you prefer no contact, please reply ‘No’.

  • Educational Information specific to patient’s disease process:

Example: “Hi Jessica, this is Gen by Gen Health. We wanted to share some helpful information regarding managing your diabetes. Key steps include monitoring your blood sugar levels regularly, following a balanced diet, staying active, and taking your prescribed medication as directed. If you’d like additional resources or have questions, reply ‘Yes’ to receive more details.

3- Message Frequency:

Message frequency may vary depending on the type of communication. For example, you may receive up to 10 SMS messages per week related to your [appointments/ personalized recommendations, etc..

Example:
“Message frequency may vary. You may receive up to 10 SMS messages per week regarding your appointments or account status.”

4- Potential Fees for SMS Messaging:

Please note that standard message and data rates may apply, depending on your carrier’s pricing plan. These fees may vary if the message is sent domestically or internationally.

5- Opt-In Method:

You may opt-in to receive SMS messages from Gen By Gen Health in the following ways:

  • Verbally, during a conversation

6- Opt-Out Method:

You can opt out of receiving SMS messages at any time. To do so, simply reply “STOP” to any SMS message you receive. Alternatively, you can contact us directly to request removal from our messaging list.
