They can simply pick up the phone and speak to their respective nurse who is well-informed of their current illness and plan.
Regardless of what it may be, it comforting for patients to know they always have someone to turn to whenever they need something.
For example, imagine a patient starting to feel immediate symptoms of their chronic illness, Most of the time, one won’t be able to reach out to a doctor to ask for clarifying questions or get advice, But with the CCM team in place, the patient can easily reach out to them whenever and be guided from there.
